There are several common vascular disorders that one may suffer especially if someone has a history of co-existing Diabetes, Obesity, Smoking, High Blood Pressure, abnormally High Cholesterol levels etc.

The vascular diseases may present in the form of
– abnormal bulges in the blood vessels also known as aneurysms which may be present in the great blood vessel aorta in the chest or abdomen also called Aortic Aneurysm.
– blockages in which the blood vessels are unable to carry a sufficient amount of blood forward resulting in the reduction in the supply of nutrition & oxygen to the tissue the blood vessel was supplying the blood to.
– Varicosities found in varicose veins of the lower limbs, in which the blood vessels become abnormally engorged & incapable to carry blood upwards towards the heart.

All these common vascular diseases can be managed safely by medical therapy or by surgical procedures if detected in time. But, if not managed well, they can result in non-healing ulcers, amputations, disabilities or even death in worse cases.

Metabolic Surgery is a procedure that has been proven to be useful in reversing Metabolic Syndrome.

Metabolic syndrome is the new name for a set of health conditions that increases the chance of developing diabetes heart disease and stroke.

It is important to have at least three metabolic risk factors to be labelled with metabolic syndrome.

  • A large waistline is found in abdominal obesity or “having an apple shape.” Excess deposition of fat in the stomach area is a greater risk factor for diabetes heart disease & stroke than excess fat in other parts of the body such as on the buttocks thighs etc.
  • High blood triglyceride level which is a type of fat found in the blood.
  • Low HDL cholesterol level which is also called “good” cholesterol. HDL helps remove cholesterol from your arteries. A low HDL cholesterol level increases your risk for heart disease.
  • High blood pressure is the condition in which the force of blood pushing against the walls of your arteries is increased as your heart pumps blood. If this pressure rises and stays high over time it can damage your heart and lead to plaque build-up.
  • A high fasting blood sugar of mildly high blood sugar may also increase the chances of heart disease or stroke.
  • The treatment of Metabolic Syndrome when done by a surgical method is called Metabolic Surgery.

Metabolic surgery

The American Society for Bariatric Surgery (ASBS in 2009 changed its name to the American Society for Metabolic and Bariatric Surgery (ASMBS) in order to promote awareness on the benefits of surgical procedures for weight loss in treating metabolic diseases especially adult-onset diabetes or Type 2 Diabetes Mellitus (T2DM).

Today the term Metabolic Surgery is used to describe surgical weight loss treatments and procedures to treat metabolic diseases especially T2DM (type 2 diabetes mellitus).

In obese patients with Body Mass Index (BMI) more than 35 surgeries like gastric bypass and biliopancreatic diversion have resulted in control/ cure of diabetes.
However, in non-obese patients with diabetes Ileal Interposition surgery or Duodenojejunal bypass surgery has been found to be useful. This can be performed in thin type 2 diabetics with BMI as low as 30 or with sleeve Gastrectomy in overweight/obese diabetics.

Bariatric surgery (or weight loss surgery) includes a variety of procedures performed on people who have obesity. Obesity is defined as a condition in overweight people who have BMI (Body Mass Index) of more than 30. This value can be calculated by dividing one’s weight in kilograms with a square of height in meters. Weight loss is achieved by creating a negative calorie balance which can either be achieved by restricting one’s caloric intake in the form of food or increasing the caloric expenditure by physical activity or exercise for a sustained period.
In bariatric surgery, the reduced intake of calories can be achieved by reducing the size of the stomach the storage bag of food with a gastric band or through the removal of a portion of the stomach (sleeve gastrectomy or biliopancreatic diversion with duodenal switch) or by resecting and re-routing the small intestine to a small stomach pouch (gastric bypass surgery).
